PAYMENT TERMS & CONSENT
This payment service is provided for your convenience by United Farm Family Mutual Insurance Company, UFB Casualty Insurance Company, and United Farm Family Life Insurance Company dba Indiana Farm Bureau Insurance and its third party vendor, Online Resources, Inc. (collectively, "we," "us" and "our"). Please read the following payment terms and consent carefully.
Payment Errors. We are responsible for initiating payments according to your instructions and you are responsible for all payments you authorize using this service. Please notify our Customer Service Center immediately if your funding information has been accessed, used, or disclosed without your permission. We will have no liability for any erroneous payments.
Email Confirmation. If you request “email confirmation,” you are consenting to electronic receipt of payment confirmations at the email address you provide. You are confirming that you have Internet access as well as a current and valid email account.
Disclaimer of Liability and Indemnification. We do not warrant that this payment service will be uninterrupted or error free. We will not be liable for any loss or damage in connection with the performance of this service or arising out of your inability to access the service or from problems with your equipment or telecommunications provider or any other occurrences beyond our reasonable control. You agree to indemnify, defend and hold us harmless from any liability, loss, claim, and expense related to your access, use, inability to use, or misuse of this payment service.
Credit Card Authorization. By entering your credit card information you are stating that you are an authorized user of the credit card and that the funding information is accurate and you authorize us to charge the amount you have requested to your credit card. You are responsible for updating us about any credit card or expiration date changes.
EFT Authorization. By entering your banking information, you are authorizing us to initiate an ACH debit from your bank account. We are not responsible for any charges or fees from your bank. A deduction that is dishonored by your bank may result in cancellation of insurance for nonpayment of premium.
Consent to Electronic Transactions. Your consent applies only to electronic payments made using this payment service. You acknowledge that you have no obligation to make payments electronically and you understand that you may make payments by paper transaction, if you wish. By confirming your credit card and/or banking information, you affirmatively consent to be bound by your authorization to initiate payment from your selected funding account(s). You may withdraw your consent to make an electronic payment only if we have not taken action in reliance on your consent. However, your withdrawal of consent will not diminish the legal effectiveness or enforcement of any transaction agreed to prior to your withdrawal of consent.
